Workshop Leader Registration for Minnesota Bluegrass festival

Please fill out all the registration information. Your check will be made out to the name on the registration which must match your W9 and the W9 must be received before payment will be made. 

W9

1. If you have never submitted a W9 to Minnesota Bluegrass, you must email a completed and signed document to info@minnesotabluegrass.org. The blank document can be found at https://www.irs.gov/pub/irs-pdf/fw9.pdf.

2. If you have previously submitted a W9 but your information has changed, you must email a new W9.

3. If we have a current W9 on file, you do not need to submit a new one. If you are unsure if we have current information, you can email one to info@minnesotabluegrass.org.

Please note that you may be subject to IRS penalties for failure to provide a W9 to Minnesota Bluegrass.
